SC/ST category candidates should attach attested photocopy of Caste Certificate issued by the competent authority. 2. OBC (Non-creamy layer) category candidates should attach attested photocopy of Caste Certificate issued by the  competent authority in the prescribed format (Obtained 06 months prior to the advertisement date) 3. Candidates are requested to check their eligibility criteria with regard to age,Qualification and experience against the advertisement etc.
Position Details	1) Medical Officer on General duty (On tenure appointment of 5 years) Requirement: No. of Posts – 9 (4 for Raebareli & 5 for Mankapur) Essential Qualification: MBBS Emoluments:  In Grade 2 scale of pay of Rs.8600-250-1460 (Pre-revised), plus other allowances as applicable to other officers in the company. Gross emoluments at Rs.23,000/- per month (approx). Age limit: 28 years (Relaxable by 5 years for SC/ST/PWD & Ex-servicemen and 3 years for OBC (Non creamy layer) in accordance with the Govt of India Guide lines. 2) Medical Officer( as a Visiting Consultant ) ENT specialist & EYE specialist  Requirement –  EYE Specialist – 2 (One each for Raebareli & Mankapur) ENT Specialist – 1 for Raebareli Visiting Hours:  Twice in a week from 9 A.M to 12 Noon on working days Essential Qualification:  MBBS with PG degree / diploma in the area of specialization Emoluments: During the period of engagement as visiting consultant, an amountof Rs.750/- per visit will be paid
